Overview: This article will provide an overview of the 'AI Care Coordination' feature

AI Care Coordination is designed to reduce the administrative burden and help practices close the loop on patient referrals. Managing referral follow-ups is often manual and time-consuming. AI Care Coordination works with patients to make appointments and helps navigate any obstacles they might encounter, ensuring continuity of care without extra work for your team.

How It Works: Before the Appointment

The system proactively engages patients to ensure the referral process is moving forward

• 72 Hour Check-In: Three days after a referral is sent, the patient will receive an automated text message confirming if they have scheduled their appointment, and, if so, the confirmed date. This status is updated instantly in PicassoMD.

What If the Patient Has Not Scheduled an Appointment

If the patient hasn't scheduled their visit, the AI Care Coordinator will provide immediate next step options:

• Direct Contact: Patients receive the specialist's contact information to call the office.
• Patient Navigation: If the patient prefers to see a different specialist, they are routed back into the patient navigation workflow to find a better fit.
• Cancellation: Patients can choose to cancel the referral if it is no longer needed.

How It Works: After the Appointment

Once the appointment date has passed, the AI Care Coordinator will follow up to confirm attendance and gather feedback on the patient’s experience with the specialist.

• Follow-Up Text: The day after the scheduled appointment, the patient will receive a text asking them to confirm they attended the appointment.
• Feedback: Patients will also be asked to share feedback on their experience with the specialist.

AI Care Coordination is not enabled for adolescent patients aged 10-17.
