Plan Before / After and Chat Curbsides
Overview: At the beginning and end of every curbside you initiate, we ask you to provide a "plan before" and "plan after". We ask for this information to better understand how the curbside impacted your treatment plan for the patient. With the exception of pharmacy, all curbsides have the same "plan before" and "plan after options".
Choosing a Plan Before:
When choosing a plan before a non-pharmacy curbside, please keep this guidance in mind:
- Send to Emergency Department - you are planning to recommend the patient go to the ED for immediate evaluation
- Refer to Specialist - you are planning to send the patient to see a specialist or to an urgent care center for a consultation or testing
- Manage in Practice w/ Care Plan - select this option if you are planning for anything other than sending the patient to the ED, urgent care, or to see a specialist
Choosing a Plan After:
- Choosing a "plan after" is similar to choosing a "plan before", with the exception of "Manage in Practice w/ Care Plan".
- Tapping this option for both the "plan before" and "plan after" will result in a second set of options. You can convey that the advice you received from the specialist either validated your initial care plan or revised your care in some way.
Pharmacy: Choosing a Plan Before / After
When choosing a plan before or after a non-pharmacy curbside, use the following information as guidance:
- Med Dosage - Ask questions regarding medication dosages or titration changes; especially helpful for pregnant women and pediatrics!
- Med Side Effect - Understand side effects, so you can educate patients or find alternatives if the patient cannot tolerate the medication.
- Drug-drug interaction - Before starting a patient on a new medication ensure there are no drug interactions with the patient's current med list or if concerned a patient is experiencing a drug-drug interaction.
- Polypharmacy concern - Conduct a holistic review of the patient's current medication regimen, including a review for medication duplication, medication underuse, or potentially inappropriate medications. (NCQA Quality Measure).
- Other - Use this option if none of the above scenarios fit your patient's case. Choosing this option will take you directly to the Curbside chat where you can begin to type your question to an expert pharmacist.
Just want to chat?
- If you are messaging a provider to send or receive an update on an existing patient or to ask a general question not related to a specific patient, rather than selecting a plan before curbside you can tap "Chat (not a curbside)" at the bottom of the screen. In this case, you will not be asked for a plan after your conversation is over.
Note: there is no chat function when you do a First Available Curbside. "Chat" only exists when you do an "Individual Provider Curbside".
